CORONARY ANGIOGRAM (ANGIOGRAPHY) IN IRAN: ALL YOU NEED TO KNOW
Coronary angiography or heart angiogram is an invasive diagnostic procedure to examine coronary arteries. It is used to discover how much your arteries are narrowing. Coronary angiograms are among medical procedures that are called heart (cardiac) catheterizations. This procedure is one of the most popular types of cardiac catheterization. Both diagnosis and treatment are considered in coronary angiography. In Cardiac angiography a special liquid dye (contrast material) and X-rays are applied to see how blood flows through the arteries in your heart. The X-ray machine rapidly takes a series of images (angiograms) then a look at blood vessels is resulted. If necessary, your doctor can open clogged heart arteries (angioplasty) during your coronary angiogram. Most of the time the access point is arm however groin may also be used.
Echocardiography lab
Echocardiography is a test in which moving images of the heart are made using sound waves. You do not need to be hospitalized to have a heart echo. Cardiac echo is not painful and is not a surgery. Echocardiography is a test used to help your doctor diagnose your heart problems or how your heart is working. Echocardiography may be necessary in the following conditions:
When you have abnormal heart sounds.
When you have unexplained chest pain.
When you have a congenital heart defect.
When you have a heart attack.
When you have rheumatoid arthritis.
How is echocardiography (echocardiography) performed?
You sleep on the bed on your side or back.
The doctor puts a special gel on the probe and transfers it to your chest area.
Ultrasound produces images of the heart and its valves.
X-rays are not used in echocardiography.
Your heart rate can be seen on the video screen.
You can take a video or a photo.
You can also watch the movie while experimenting.
This test often takes less than 15 to 20 minutes.
This test is painless and has no side effects.
Your doctor will talk to you about the results.
Echocardiography is located in the heart clinic Alzahra specialized and sub-specialized hospital.
Angiography
Coronary angiography is an X-ray imaging technique used to examine the coronary arteries of the heart. Usually, when blood flow to the heart muscle decreases, your doctor will use angiography to find out which of the arteries supplying the heart are blocked. In coronary angiography, a dye is injected intravenously into your body, which makes your arteries visible using X-rays. An X-ray machine or angiogram provides an overview of the condition of a clogged artery by taking continuous pictures of the arteries. If needed, a specialist doctor can open blocked arteries during angiography, which is called angioplasty.
What is the required time for coronary angiography?
The coronary angiography procedure usually takes 1 to 2 hours.
What are the needed paraclinical actions before doing coronary angiography?
Before having an angiogram, usually, you will be asked for a blood test and a chest x-ray.
Preoperative, intraoperative, and postoperative care of Coronary angiography surgery
Preoperative cares:
First, before the operation starts, the doctor will check the history of the disease and the medications that the patient is taking, and then he will examine the whole person’s body. It is best to give your doctor a list of medications, supplements, vitamins and herbal supplements you are taking. Blood-thinning drugs such as aspirin, Plavix, warfarin, dipyridamole, and any other medication should be discontinued completely in consultation with your doctor a few days before angiography to prevent uncontrollable bleeding during the procedure.
Angiography may also be performed urgently, but in general, it is common to plan ahead so that the patient is ready for the operation. Since angiography is performed in a hospital, the following points should be considered:
Do not eat anything from midnight before the surgery
Take the medications you are taking with you to the hospital and ask your doctor about their use on the day of surgery.
If you have diabetes Tell your surgeon and operating room staff. Because if you are taking a metformin drug, more measures are needed to protect your kidneys from the injected substance.
If you wear contact lenses or glasses or carry items such as jewelry, give them to the receptionist of the hospital. You will also be asked to empty your bladder and put on special clothing.
Intraoperative care of Coronary angiography surgery:
You will lie on your back on the bed after entering the angiography surgery room. An angiocatheter will be gently inserted into your vein and then electrodes will be placed on your chest to check the condition of your heart at any possible time. Also, a Blood pressure measuring cuff and a device for measuring blood oxygen levels will attach to you.
Angiography does not require general anesthesia, but a sedative will be injected into you to help you relax during the procedure.
Next, the hair in an area of your body where a catheter or thin plastic tube is to be inserted into the arteries will be shaved and disinfected with betadine solution. The area where the catheter enters your body is in the groin, arm or wrist. Do not get worried because of the pain of the catheter entering the body. Local anesthesia helps you not to feel pain. After injecting a local anesthetic drug, a small incision will be made in the skin and a catheter will be inserted into a vein. The catheter is guided by a doctor to the coronary arteries. There is no pain when the catheter enters the body and passes through the body, and you cannot even feel its movement. Note that if you feel uncomfortable during the operation, notify the treatment team immediately.
Following the operation, a special dye will be injected into your body. It is normal to feel hot at this time, but if this feeling turns into pain, be sure to tell your doctor. When the dye is injected, the coronary arteries will be visible and the doctor will be able to identify areas of the vessel that are blocked.
